Office Hours & Doctors Schedules

Our office hours are 7:30 AM to 5:00 PM, Monday -Thursday, and 8:00 AM to Noon on alternating Fridays.

Providers are at lunch from Noon to 1:00 PM.

Our new schedule is as follows.

Dr. Waltemath is here Monday - Thursday from 7:30 AM to 3:00 PM.

Dr. Keenan is here Monday -  Thursday 1:00 - 5:00 PM;

Dr. Parkes will be here Monday - Thursday 8:00 AM - 5:00 PM, and on alternating Fridays he will be here from 8:00 AM - Noon.

In case of a dental emergency, please call the office (615) 298-2040 and you will be given instructions about what numbers to call.  Someone is always on call when the office is closed.  We do not want patients to suffer any longer than necessary in emergencies.

  Cancellation Policy

We do ask our patients to give us a 48 hour notice, when possible, if you need to cancel an appointment.  Patients who habitually cancel appointments are subject to a failed appointment charge of $50.  Our office does not double book.  We reserve time especially for you.

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete the appointment request form below. Our scheduling coordinator will contact you to schedule your appointment.  When you have schedule your appointment we will give you a patient ID number for you to go to E-Forms on line (See our new patient forms on-line button) and fill out your required new patient information.  We need all the information filled in and you have  until two hours before your appointment to complete the forms.  We can then pick it up on-line and print it out so you will not have to remember to bring it with you to your appointment.
